Congestive heart failure (CHF) is one of the leading causes of hospitalization for older adults. Readmission among congestive heart failure patients can be as high as 35%
In the US, 5.7 million adults have heart failure, costing the nation an estimated $30.7 billion each year
Shift post-acute care into the home
Evidence shows intensive interventions, like remote patient monitoring, keep recurrent high cost utilization patients out of the hospital. These technology and healthcare solutions enable the patient’s care team to monitor their patient’s health. All these opportunities must be integrated into the eNable platform.

This project focused on chronic disease management and CHF was chosen as our first use case. To help the cross functional teams at Altais understand our target audience, I created a new persona.
I created a new persona card with easy to use Figma components as a resource to create a new standard for the team. This made future persona creation more efficient.

Strategic ideation sessions
Several brainstorming sessions were facilitated by myself and the other senior designers to guide key stakeholders from the engineering, data science, and clinical teams including our chief medical officer to understand how to deliver the most effective experience for our users.
Key information captured included modality, data and AI integration, interoperability requirements, and end-to-end workflows.

Feature list
A list of ability to statements was distilled down from what was learned during the design sessions. I maintained the status of these features as the team worked through them to provide transparency.
